Full Job Description
Position/ Title- Clinical Pharmacist & Healthcare Advisor
Purpose of this position-
- Achieving desired/targeted outcomes for patients registered with the company’s Medication Therapy Management Program and the customer satisfaction that will ensure high levels of patient retention and patient referrals.
- Serve as domain specialized knowledge lead for guidance of patients in their health care journey as well as owner/support for internal processes
Key Accountabilities/ Responsibilities-
- Progress of patient on key vital parameters or any unified score. Customer Satisfaction Rating for visits made to customers.
- Identifying and resolving gaps in prescriptions in a preventive manner in consultation with patient and their doctor
Day to Day responsibilities-
- Responsibilities include the following and other duties may be assigned
- Explain the features of the Relex Medication Therapy Management Program to prospective or existing customers/patients and obtain referral for patient to join the program
- Scrutinize and digitize patient prescriptions and ensure thorough analysis and vetting before medication is dispensed
- Patient counselling and guidance on correct use of medication for achieving desired/targeted outcomes. Provide information and explanation of diagnoses, options, outcome and risk and escalate where required
- Conducting patient evaluation Doctor Virtual Connect program
- Closely monitoring patient progress through regular review patient outcomes and data, follow up on lab tests and their results. Intervention with patient and/ or patient’s doctors where necessary.
- Support customer outreach programs through conduct of counselling and tests associated with theme of the program
- Communicate customer feedback and customer experience on products / services and/or modifications to existing products/services to Operations, Marketing and Quality teams.
- Data updation in all tracking platforms on a daily basis
Competencies / Skills Required-
Customer Orientation:
- Etiquette, Empathy, listening skills and good bedside manners
- Disease and applicable Pharma knowledge
- Good written and verbal communication including reporting accuracy
- Adherence to time schedules and processes including safety procedures
- Must have the ability to learn and retain complex material, pass tests demonstrating product and diseaserelated knowledge and deliver content to Customers verbally in a clear, effective and accurate manner in strict c ompliance with regulatory and other requirements.
- Ability to read, comprehended and explain medical reports and prescriptions in relation to the Product / Service provided.
- Ability and willingness to travel locally
- Requires time flexibility as necessary to meet customers’ schedules and coordinate accordingly
- Detail oriented, even when faced with highly repetitive tasks
- Must be able to work both with external and internal customers.
- Comfortable in new environments and challenging healthcare conditions. Must be able to work and interact with diverse groups of people.
Educational Qualifications : Doctor of Pharmacy ( Pharm D)
Desired: Experience in Clinical Pharmacy functions in a good healthcare establish ment and a useful base experience of patient counselling on medication
Work Experience (Relevant and Total Experience – Fresh graduates to 1 year experience
Technical Qualifications-
- Should have a valid Driving Licence
- Should have a two wheeler with valid documents such as vehicle RC Book, Insurance, Emission Test and vehicle Insurance.
- Registration with the Pharmacy Council of concerned state where deployed on duty
